0215 (2): Several staff do not have record of medication training. Seizure medication on site without staff in classroom at time of visit being trained on administration of this drug. Two staff include Eleanor Albrecht and Carvon Stout both in ratio in classrooms. 0215 (3)(a)(i): Seizure medication on site without medical authorization form. 0215 (3)(a)(iii): Several diaper cremes present at Center without medical authorization forms.
